After you have scheduled,
you will be sent a packet in
the mail that contains
Medical History, Dental
History, Office Policy,
Patient Information, an
appointment card with your
scheduled time and a map to
the office. You will be
asked to fill out these
forms and bring them with
you when you come in for
your initial exam.
We will need either to take
a full series of x-rays or
request them from your
dentist, if you have had a
set taken recently. If you
will be getting x-rays from
another dentist, we will
need them prior to your
appointment. If you have a
referring dentist and do not
have x-rays, we will send a
set to them with a report of
Dr. Sharp’s findings and
suggested treatment plan.
When you come for your
initial exam, one of Dr.
Sharp’s assistants will go
over your medical & dental
history with you. You will
then see Dr. Sharp for the
exam. In approximately a
week after Dr. Sharp has
evaluated your exam
findings, you will come back
and discuss his findings and
proposed treatment.
At that time he will answer
any questions that you might
have. A spouse or friend may
accompany you for this
discussion.
We expect 24 hours notice of
cancellation if you cannot
keep an appointment. |
